    • Health Maintenance Exam Every Winter To Review Appropriate Screening Guidelines And Ensure Your Future Health Is Not Falling Through The Cracks

    • Example: Screening colonoscopy, Pap Smear, Mammogram, PSA for Prostate Cancer, Dexa Scan, etc…

  • All Other Visits

    • 30-60 Minute Long Visits available

Payment Structure

Traditional Membership

  • One Time Registration Fee: $100

  • Monthly Fee: $100

    *(Ex: Pay $100 Registration Fee when starting Membership. The Monthly Fee billing of $100 will begin 1 month after Registration Fee is charged, so you will NOT be charged $200 at the start of Membership.)

  • Annual $1,200: If You Pay All 12 Months At Signing, The $100 Registration Fee Will Be Waived

OMT Only Membership

  • Each OMT Visit is 30 Minutes

  • $50 for First OMT visit

  • $35 for follow up OMT Visits

Traditional Membership For Families

  • Each Child (<18 Years Old) Is Eligible With Registration Of At Least One Adult Parent/Guardian

  • Adult Pricing As Above

  • No Child Registration Fee

  • Child Fee: $25 Per Child Per Month/Annual $300 Per Child
